1. Go through the four case studies by clicking on Program Menu and selecting one
2. Ask any questions you may have and then request to take the quiz from within the case study
3. I will email the quiz URL (internet address) to you. You do NOT receive the quiz automatically 
   so please do NOT request the same quiz 10 times. Give me a chance to reply. Please do NOT give the URLs to others. [Note: I will be unavailable to send quizzes from June 11 - 19]
4. Take the quiz ONLINE.
5. Submit the form ONLINE for all the case studies. (Click on Form and Eval above). We can also FAX your certificate if you indicate your FAX  number on the form. (However, I don't have time to fax certificates if I have to call you up first to tell you to turn on your computer and not answer the phone!)
6. Send us the $40 fee for 4 hours. If you want your certificate sent by Federal Express, please send us the pre-paid return Fed Ex  envelope when you send in your fee. Make your check or money order out to the Dept. of Respiratory Care Education and send it to:

7. When we receive your fee we will mail (or fax per request) you the certificate. Allow two weeks  for the whole process. It may be done in a week if you follow the Instructions. Please note: We are not responsible if your email is Undeliverable or if you don't know how to use a computer, email, or your internet browser. We are not responsible if you do not allow two weeks from the time you mail your check to receiving your certificate by mail.
8. Your failure to read and follow these directions does not constitute an error on our part.

Please note:

	If you do not supply your correct, complete email address when you request a quiz, I cannot send  you the quiz to take.
	If you do not supply your complete phone number including area code with your request, I cannot  call you to tell you that your email address is incorrect. I do not guarantee calling every participant  who supplies an incorrect email address.
	If you do NOT intend to complete the CE process, do NOT request to take the quiz.
	If you have requested a quiz and do not receive it within 48 hours, your email address was  incorrect or undeliverable for some reason and your phone number was incomplete.
	If you fail to submit a passing score on the quiz, please review the case study and take the quiz again.
