Objective: Identify at least three educational strategies/activities that will support health-related changes for individuals with psychiatric disabilities.

1. Review the following article from your initial reading assignment:

2. Working with your Small group: Prepare a 1 page synopsis that outlines:

1. What learning activities might be used for your session topic?
2. How do these learning activities address issues related to the underlying concepts of the Stages of Change Theory and Social Cognitive Theory?
3. How will these activities enhance empowerment with individuals with SPMI?

Prepares 1 page synopsis on health promotion activities for session topic.

6 = Synopsis no longer than 1 double-spaced page, clearly written with sources cited and references in APA format.
3 = Synopsis is within page limit, contain few editing and spelling error or errors in using APA format for citations and references.
0 = Synopsis is longer than 2 double spaced pages, poorly written and sources are not cited or referenced in APA format.

Proposed activities are logically linked to the Stages of Change Theory individual model of health behavior and appropriate for the target population.

9 = At least three (3) activities are identified and logically linked to individual model of health behavior and appropriate for the target population.
5 = At least three (3) activities are identified and logically linked to individual model of health behavior but not appropriate for target population.
0 = Identified activities are not logically linked or appropriate for target population.

Identified activities enhance empowerment with individuals with SPMI.

9 = Relationship between activities and empowerment are clearly explicated.
5 = Activities are appropriate but relationship with empowerment are not clearly explicated.
0 = Activities are inappropriate for empowerment.

6 = Synopsis clearly indicates that activities are consistent with underlying concepts of the Stages of Change model of individual health behavior and Social Cognitive Theory.
3 = Synopsis marginally demonstrates that activities are related to the underlying concepts of the Stages of Change model of individual health behavior and Social Cognitive Theory.
0 =Activities are independent of theoretical models.
