Now read the analysis of these problems, paying attention to what data was used. Was the data collected the best measure? What data sources (persons, chart information, revenue and cost utilization) that should be added. Review each team's action plan and think about the "effort" or "work" that is added by the new plan compared to the expected outcome of the plan described in the results.

Evening shift increased filing, but this first action plan did not get results. Work with v.p. for long term funds. Increased MD and staff satisfaction, color code jackets, retrieve films by date of service now.
Identify common needs of patients, consolidate for observation of patients, clarify new locations, need rehabilitation unit. Patient placement guidelines worked well except during high census. Need overflow plan. Place all specialty and high risk patients on same unit. Combine med/surg units, patient stays on unit admitted. Cross train and eliminate staff.
Root causes: No formal dietician orientation, outdated information passed on, no teamwork, "that's your job, not my job." Welcome breakfast for orientation. Accountability chart developed and shared. Policy and procedures adopted, supervisor audit to month; new employee review, patient fed and happy.
