  Sample APN Portfolio Introduction

Philosophy:  I chose nursing as a profession 33 years ago with the goal of helping people help themselves to stay as healthy as possible (see Curriculum Vitae Attachment A). Today, my approach with my clients and families continues in health promotion of bio-psycho-social and spiritual wellness to help them improve their quality of life for themselves and their significant others.  

Nursing Role:  The ideal role I would like to cultivate in the next five years is a dual role as a clinical nurse specialist in an independent weight management practice and in a collaborative practice with one or more health care providers. Since earning my doctorate in nursing, I have practiced and conducted research in long-term weight management, specializing in weight cycling in female clients (see Flyer, Attachment B). I developed the Holistic Self-Care Model (see Dossey’s Holistic Nursing Practice Chapter 27, Attachment C), which guides my practice—total assessment and care for well being of individuals’ mind-body-spirit. Clients are ultimately responsible for carrying out our negotiated approach to long-term lifestyle changes in eating, exercising, and relationships. I act as a personal coach and resource to help them experiment and refine healthier lifestyle habits that promote health.  

Collaboration:  I do not prescribe drugs nor conduct specialized assessments and planning for clients’ co-morbid conditions. I seek to collaborate with a health care provider or bariatric physician to refer clients with these concerns. My collaborative role would be to continue overall care in relation to long-term weight management.

Contract:  I seek a five-year contract with at least one other health care provider who practices with clients with overweight concerns.  Ideally, I wish to begin Year 1 with a base salary of $ XXXX for 50% of my services and the remaining 50% would be a negotiated combination of direct billing (see flyer for fee for service Attachment C) and “incident to” billing under another care provider (100% of MD allotment). I will apply for my own Medicare provider number and plan to submit my own billing in Year 2, when I would bring 85% of MD allotment to the practice along with 100% fee for service receipts. Also in Year 2, I would expect my base salary to be reduced by 25% and totally discontinued by the end of Year 3. Total income would come from direct fee for service and 3rd party billing in Years 4 and 5, at the end of which time the contract could be re-negotiated. All negotiations may include profit-sharing.
