URI Cases

Cut and Paste (or Print off) the following to a word document.  Select two cases from the following seven.  For each of those cases complete the following (create your own additional details as needed for the patient). Complete and  Email responses to instructor (or bring to class).

1. Write up three likely/possible differentials for the chief complaint or main presenting problem.
   

2. Identify five relevant history questions to elicit further information. 
   

3. Identify five “don’t miss” components of the focused exam.
   

4. Identify any plans for lab or x-ray.
   

5. Identify any recommended prescriptions for your most likely differential.
   

6. Identify any recommendations to patient including patient teaching points.
   

7. What will be the follow-up plan?

AM patients

1. A 40 year old male patient comes in, “I have problems with allergies every year at this time”

2.  A 53 year old female, “I’ve had a cold for the past ten days and now my chest feels tight.”

3.  A 63 year old female with history of HTN comes in for a cold. BP elevated at 148/90. 

4.  A 23 year old male with dry hacking cough, myalagias, malaise, sore throat. Started abrupt yesterday. 

5.  A 44 year old female with “bronchitis” who reeks of smoke. 

Calls from the nursing home:

1. Staff report Mrs. Jones hasn’t been her usual self for the past 2 days. She’s not eating well. Sleeping a lot. Seems like she has a cough once in a while.

2.  Family reports, “Dad isn’t eating well. That stuff he’s always coughing up used to be clear, but now it’s yellow-green.”
