

Objectives:
Describe the impact that demographic trends (with regards to elders and persons of all ages living with chronic illness) will have on our health care system.
Using the Dimensions Model of Health, describe normal & anticipated health changes for elders. Identify appropriate primary, secondary, and tertiary interventions.
Describe personal and population effects of chronic physical/psychological health problems. Identify appropriate primary, secondary, and tertiary interventions.
Describe biophysical, psychological, physical environmental, socio-cultural, behavioral, and health system factors that influence the health of the elderly population.
Describe biophysical, psychological, physical environmental, socio-cultural, behavioral, and health system factors that influence the development of chronic health problems.
Using exercises provided, plan health care and life transitions for both chronically ill populations and elders.
